OFFICE INFORMATION

THANK YOU FOR CHOOSING DR. RANDALL HAYS AS YOUR PRIMARY CARE PHYSICIAN.

 

TO ASSURE THAT YOUR HEALTH CARE NEEDS ARE TAKEN CARE OF, PLEASE FIND BELOW INFORMATION THAT WILL ASSIST US WITH YOUR CARE IN A TIMELY MANNER.

 

• OFFICE HOURS --- We are open Monday thru Friday 8:00 am to 5:00 pm.

 

• AFTER HOURS / WEEKEND EMERGENCY --- After hours calls are paged to your doctor or the doctor on call. ONLY EMERGENCY HEALTH SITUATIONS will be addressed- this does NOT include prescription refills or minor symptoms and questions that can be addressed during normal office hours. If you have a life-threatening emergency, please call 911 or go to the nearest Emergency Room, following up with Dr. Hays in the office as soon as possible.

 

 

• MEDICATIONS / REFILLS --- Routine refills must be requested by your pharmacy via fax to our office at 214-387-8395. Whether or not prescriptions will be refilled without an office visit is at the discretion of the physician based on each patient’s medical history and most recent office visit.

 

• REFERRALS --- Referrals to other physicians are based on medical necessity and can not be accomplished without an office visit to review the clinical information required for referral. Every health insurance company has different requirements for referrals. We will work to complete your referral as quickly as possible, but it may take up to 3 business days and reasonable planning is requested on the part of the patient.

 

 

• MISSED APPOINTMENTS --- Please contact our office 24 hours in advance if you are not able to keep your appointment. This allows us to take care of the needs of other patients. Repeated missed appointments without calls to cancel are subject to No-show fees.

 

• LATE APPOINTMENT ARRIVAL --- We will make every effort to work you back in to the schedule if you arrive late for your appointment. Please be advised that it may also be necessary to reschedule your appointment based on time availability.

 

 

• INSURANCE --- It is the patient’s responsibility to provide a picture ID, insurance information that reflects the correct Primary Care Physician, and contacts to verify coverage and billing address. We can not provide medical treatment utilizing your insurance benefits without the complete and accurate information described above.

 

• PAYMENT --- Insurance co-payment, plan deductible, or payment in full is expected at the time of service. Payment arrangements may be available for our patients with large deductibles. For other billing concerns or questions, you may contact our billing company at 1-888-238-9671.
